When you create a form, you will be provided with one sample question which you can change as required. For each question, you can enter specific text and set the question type e.g. multiple choice or a date or time. Once the sample question has been changed, you can add any other questions required.

When you create a form, you will be provided with one sample question. To change this sample question to the first question you wish to ask:

If you choose Multiple Choice or Checkboxes, you will be able to enter all options to appear as radio buttons or check boxes for the question. Change the text of Option 1 as required, then click on Add Option to add another choice. To remove a choice no longer needed, click on the X after that choice in the list.

If you wish users to be able to add their own text to to a multiple choice question, click on Add "other" to add this option.

If you choose the Drop-down question type, you can enter the first option to appear in the list, then click on the Add Option link under this to create another list item.

If you choose the Linear Scale type, you will be able to choose how many option buttons to show in the scale, as well as the labels for the first and last option in the scale. 

If you choose the Multiple-choice Grid or Tick Box Grid type, you can choose the number of columns of choices that will be presented, and can type the heading to appear at the top of each column of radio buttons. Type each question as a row label.

Once you have created the first question, additional questions can be added using the + button on the side of the form.
