In a Google form, you can now allow anyone filling in your form to submit files via an “upload file” question. The file(s) they upload will be stored in a folder on your Google Drive, and a link to the file will be added to the response sheet of the form.
To add a File Upload button to a form, the form must only be accessible to users within your domain.
To set this:
Click on the Settings icon near the top-right corner of the form
Check the box to Restrict to (your domain) users
Click on Save
You can now add a question to the form that allows viewers to upload a file.
Click on the Add Question button
Choose File Upload from the list of question types
Click on Continue
Enter a question name
Choose the maximum number of files a user can upload
Choose the maximum file size a user can upload
To limit the file types a user can upload, click on the Allow only specific file types slider and check/uncheck all options as required
You can add as many upload file questions to your form as required. Each can be used to upload files to a separate Google Drive folder.
When a viewer opens your form, the question will show a button that they can click on to upload a file from their computer or Google Drive.
To view the files uploaded via your form:
Click on the Responses tab
The file will be listed in the responses - click on the View Folder link to view the folder in Google Drive
