You can add images a form, to support the questions you are asking on the form. If you are using a form as a test, for example, you could include an image on the form, then ask specific questions that relate to that image. You could also offer a list of images as multiple choice answers to a question.
To add an image to a question:
In a form, click on question you wish to add an image to
Click on the Image button
To add an image to an answer:
In a form, click on question you wish to edit
Click on the answer you wish to add an image to
Click on the Image button
To insert an image from file, ensure the Upload option is selected along the top of the dialog box, then either drag the image onto the dialog box or click on Choose an image to upload. After selecting the image file, click on Open
To add an image by URL, click on the By URL option along the top of the dialog box, then click in the Paste an image URL here box. Type or paste the URL of the image - a preview of the image will display
To add an image from your Google Drive, select Google Drive. Choose My Drive to show all of your files, or Shared with me to show files shared with you, then choose the required image and click on Select
The images will show in the form:
Remove an image by clicking on the X of the image.
If you add an image to a question rather than an answer, you can click on the ellipses to show more options e.g. to align the image:

Preview the form to see how the images will look for questions and answers:
